Fee Information

Dr Frischtak does NOT participate with any managed care or insurance plans and does NOT accept insurance assignment (fee negotiation). You are responsible for paying your bill at the time of your appointment. 

Please contact Dr Frischtak for her fee schedule.

Dr Frischtak does print a receipt that patients can submit directly to their insurance company for possible reimbursement.

Dr Frischtak accepts checks, credit cards (Mastercard ,VISA, American Express, Discover) and cash.
