Cancellations and Missed Appointments
Please provide 48 hours advance notice – excluding holidays and weekends – to reschedule or cancel your appointment.
All missed appointments are charged full fee if not canceled within a 48 hour notification.
Cancellations due to inclement weather will not be billed. You many cancel or reschedule an appointment by email or call 888 391 5517
If you are late for your appointment Dr Frischtak will meet with you for the remainder of your appointment time. You will be responsible for the full fee for your appointment. Dr Frischtak can not go over the allotted time to "make up" for late arrivals. Please plan enough time to arrive at the appointment.
Prescriptions Refills and other Medication Requests
Dr Frischtak addresses prescription refill requests on weekdays usually within 24 hours of the received request.
To minimize errors and increase efficiency Dr Frischtak prefers to receive prescription requests in writing by email or have the pharmacy send her a fax or transmit a electronic request. Please always include your full name, DOB, medication needed and dosage and the pharmacy number.
Emergency medication refill requests - off business hours/days - will be addressed as soon as Dr Frischtak is able and may be subject to the administrative fee.
For clinical/medication questions that can be answered quickly by phone you will not be charged.
Dr Frischtak may charge you for extended phone calls/consultation fee. Dr Frischtak may ask for you to schedule an appointment. Extended phone calls are not a substitute for a in person follow up appointment.
Dr Frischtak complies with HIPPA privacy guidelines of confidentiality. She will not disclose information about you without your written permission, except in certain very specific situations where she is required to do so by law.

Urgent issues and Emergencies
If you have an Emergency please go to the nearest Emergency Room and/or call "911".
For Urgent issues contact Dr Frischtak during clinic days but also reach out to your primary doctor since at WholePersonCare we are consultants to your team. We are working WITH your doctor and other providers who should be notified of there is an urgent issue. If you can't reach any of your providers within a reasonable amount of time, please go to your Emergency Room or call "911".
Billing and Payment expectations
Payment is due at the time of service. Dr Frischtak accepts checks, and credit cards ( Mastercard ,Visa, American Express, Discover). You are responsible for paying your bill at the time of your appointment. Statements will be given to patients at the time of visit.
